sat/vB is the fee per unit of transaction size. Wallets like Sparrow, Electrum and Wasabi let you type it in. If you're not in a hurry, the “1 hour” or “economy” rate usually confirms the same day for a fraction of the cost.
Monero fees are tiny and stable: a normal payment costs a few hundred-thousandths of an XMR, so there's rarely a reason to raise the priority.
Difficulty adjustment happens every 2,016 blocks (about two weeks). A big drop usually means miners went offline; it doesn't affect your fees directly.
